Approved Survey — Blood Glucose Management and Diets Type

Hello,

My name is Angele; I’m a high school student conducting an ethics-approved science fair project on how individuals with type 2 diabetes experience antioxidants in their diet and/or the different ways they maintain their blood glucose levels.

The study includes two parts:

Part 1: an anonymous survey that assigns each participant a Participation ID: Glucose Level Management (Part 1)

Part 2: a brief 4‑day log where participants use their ID instead of any personal information. No names, emails, or identifying details are collected at any point: Glucose Level Management (Part 2)

The 2nd part contains a consent form for compliance and safety. Consent forms from participants in part 2 are handled exclusively by the qualified scientist supervising the project, as student researchers are not permitted to manage identifiable information. All data is anonymized and averaged across participants. Additional information is included within the surveys.

Respondents/Participants can only fill out both surveys once; this may require that they sign in to their account (Google, Yahoo, etc) before accessing the survey or they may see their email on the form. However, I DO NOT collect emails (I will not see them on my end). When opening the survey, they should see their email with a note saying “Not Shared” next to it.

This is a quick note to confirm that the survey was authorized by our moderation team. We tested the survey as presented to us and verified it was not collecting email data by default.

@lonelworld Angele, I did part 1 and have stopped on Part 2. The question on insulin use is MDI only, those of us on a pump only use rapid insulin for both basal and bolus. For basal it is metered every 5 minutes based on our units per hour and if the pump moderates that depending on blood glucose level data from a CGM.

For pump users, add TDD (total daily dose), daily basal and bolus, but remember it’s all one kind of insulin.

The other issue I have is with the 5 day log. I don’t keep a regular log any longer but rely on pump history, Dexcom and Apple Health app.

Hello.

Thank you very much for your feedback! I have just added an ‘Other’ section to the form under insulin use, where participants can add other types of insulin that were not listed in the selection. I will try and see if I can add the total daily dose for pump users — my project has already been approved by IRB and any significant modifications like total daily dose question would require re approval (however the deadline has already passed). However, I have included notes questions for every day log, so participants may use to include this information if they wish to do so.

About the 5-day logging:

•​You may use Your Device History: You do not need to keep a new manual log. You can simply look at your pump history or Dexcom reports at the end of the day (or even at the end of the 4-5 days) and enter those numbers into the form all at once (either in mmol/L or mg/dL).

•​Use ‘N/A’: If your device doesn’t track a specific data point we asked for, or if you didn’t check at a certain time, you are always free to write ‘N/A’.

•​Automatic Saving: You can open the form link, enter one day’s worth of data from your device history, and close it; Google Forms will save your progress for when you return.

I apologize that it took me so long to answer the second part of your question/concerns.
