List of NDC numbers for diabetes equipment and meds

Important: to help other members, please add your NDC numbers to the thread if your meds/ equipment are not listed here!

What are NDC numbers?
NDC numbers, in the US, are universal numbers that can be given are a unique reference for a specific medicine or piece of equipment. Many of us have run into difficulties ordering diabetes gear that doctors or pharmacists erroneously reference or identify.

Have more NDC info?
If you have meds or equipment that are not in the list, can you share the NDC number for it with us below in the thread? Please don’t forget to include the name, additional identifying information if any (such as, for instance, syrup vs. pill) and quantity.

Need more NDC info?
If you want the NDC number for specific meds that are not listed here, please ask the question at the end of this thread! Hopefully, one of us will have the information at hand.

Verify before getting.
Before you request a prescription by NDC, always verify the NDC is correct for what you want by looking it up here:

Please note:

  • I made this a pinned topic until October 31. Anyone can unpin it from their own screen: I think it is enough to read the whole thread.

  • Any level3 may edit this wiki. But, if you want to add meds to the list and are not sure of how to edit the list, just add your info to the end of the thread, and someone will edit the wiki.

  • Feel free to propose (or directly change) the wiki as needed for more information or clarity

Many thanks to @MM2 for all the Dexcom NDC codes!

I added a bunch of stuff and made some edits.

First of all, NDC is “National Drug Code”. So referring to it as the NDC Code is like saying “National Drug Code Code”.
(Like from the Department of Redundancy Department. :grinning:)

Anyway, I changed it to NDC number in a bunch of places.

Also, adding a reminder for everyone to verify their codes by checking them here:
https://ndclist.com/

I put some blank lines between various types of meds.

Maybe would be good to have one person modifying the list to maintain consistency and avoid duplicate entries?

I think this is all the information asked for above. I verified the NDC number at the suggested website.

@Tapestry, nice, thanks so much, that is perfect info!

Maybe another section should be added for things like SYMLIN and Metformin and so forth?

Anything else we should make a section for? We should put pods in there and infusion sets perhaps?

Test strips maybe? I feel like these can be easy to screw up, I know I’ve had a previous endo write me a script for “Contour” strips when I needed “Contour Next” strips

Yes that would be helpful, but I will tell you that there are at least 12 bajillion of those. So there would be some serious effort to put them all together.

Yes. I put it in its own section, but I figure we should probably create subsection titles as well to make things clear: insulin short acting, insulin med or long acting, other meds, supplies, equipment. Possibly more detailed if it gets crowded.

It would be great to have a test strips section too.

See what you think. A work in progress…

There are two types of Symlin one for Type 1 and another for Type 2. The difference is in the dosing and they each have their own NDC number. The one listed (quoted above) is for Type 1; I believe the other is for Type 2, at the higher dose (shown below). It can been seen here:

Great list by the way! Thank you

@Tapestry, is it correct that you are using the Symlinpen 60? I think the NDC number you gave earlier is specifically for the Symlinpen 60. I am guessing the other one is for the Symlinpen 120?

Here is a link that has links to the instructions for both pens:

https://www.symlin.com/using-symlin/how-to-use-symlin.html

Yes, I had used Symlin 60 (for a few years) and I had the number from some unused supplies. I’m no longer using Symlin, but I have a few boxes left, so I was able to get the NDC number.

Yes, I believe you are correct, the other pen is Symlin 120 and has a different dosing structure.

I was looking for @BradP the NDC for OmniPod pods (non Dash), but it isn’t showing here (neither Dash or non-Dash are showing). I did find it for both the PDM and Pods on this website:
